Registration at Cooper is available on the following dates and times:

Registration Requirements:

• Original Birth Certificate (from Bureau of Vital Statistics, not hospital copy)
  

• Child’s Social Security Card
  

• Immunization Record
  

• Proof of Residency (bill of sale, lease agreement)
  

• Last Report Card and STAAR Scores (Grades 3-5)
  

• Parent/Guardian Picture ID
  

• Pre-K enrollment requires proof of income (3 current pay stubs and child must be 4 years of age by Sept. 1, 2017)
